An online intake and waiver form will be emailed to you prior to your appointment. Please have this completed before coming. If you're having a hard time completing the form, please avoid filling out your postal code.

Our RMTs all have different styles and techniques when it comes to the art of massage. Please do not hesitate to let them know what you would like worked on. If the pressure is too much or too little, if you would like a quite appointment and if there's anything you would like adjusted on the table.

Yes, we direct bill to most insurance companies.
Patients are responsible to know and understand what their insurance policies ask of them. Plans may require a doctors note and or a deductible may be paid out at the time of your appointment.
In the event that direct billing is not possible, please expect to pay the full amount of your appointment. We will be more than happy to print or email you a copy of your receipt to allow you to self submit, and keep for your records.
We direct bill to the following insurance companies:
If an appointment gets cancelled due to unknown insurance policy there will be a $20 charge.
A cancellation notice via phone, text, or email of at least 24 hours will still be required. You are also able to cancel via the automatic email reminder we send 48 hours prior to your appointment.
If a notice is given in less than 24 hours you will be charged 50% of the cost of your treatment.
If a notice is not given and you do not show up for your appointment you will be charged 100% of the cost of your treatment.
These charges are not able to be charged through your insurance and will be your responsibility.
A credit card will also be required to book your appointment and will only be charged in the event of a no show or a last minute cancellation notice.
Street parking is available right in front of our building called CLICK FLOORING.
Our parking lot is located just past LEWIS INSTRUMENTS. Parking stalls will be located closest to the back lane with sign indicators saying "942 ERIN STREET CLIENT PARKING".
